To provide case management services to clients, families and children in Elkhart County involved with the Department of Children Services. Help clients gain access to needed medical, social educational and other services. Provide direct assistance in gaining access to services, coordination of care, oversight of the entire case and linkage to appropriate services.

Case management services include:

  • Assessment of the eligible individual to determine service needs
  • Development of a specific care/treatment plan
  • Referral and related activities to advocate and assist the individual obtain needed services
  • Monitoring and follow up of the plan
  • Evaluation (of impact of treatment plan on clients recovery)
  • Linkage and networking with DCS, court, schools, health care providers, employers and other community agencies
  • Provide integration and on-going assistance to clients and families to help sustain within the community/least restrictive environment
  • Accountable for the flow of information within a multidisciplinary team setting
  • Special Populations/Skills/Employment duties, as assigned.

Customer Contact/Relationships:



The Child/Adolescent Care Facilitator interacts directly with clients and families. The frequency varies with needs, availability of client, and level of intensity. It is usually weekly or biweekly and includes availability for crisis intervention. The employee also interacts frequently with outpatient therapists, physicians, and other center employees involved in the case.

There is regular interaction with community agencies and providers such as:
DCS, court, schools, health care providers, referrals and coordinated services. Caseload expectations will take into account training; case complexity, level of frequency and/or other assigned duties.



Minimum Hiring Requirements:



Academic:
BA/BS required. (BA/BS in social services or human services related field preferred.

)



Experience:
Two years experience in a social service setting preferred.



Must have a valid driver's license and provide proof of insurance.
